Knowledge Management Service and the Knowledge Norfolk website
The Knowledge Norfolk site supports health and social care professionals by providing information and resources needed on a daily basis to support patient care.
The site provides access to:
- Local and national guidelines and pathways
- Local prescribing guidance, include formularies, newsletters, PGDs, shared care agreements and TAG recommendations
- Service and referral criteria (including Community and Hospital services, Support Groups, Statutory, Charity and Voluntary organisations)
- Referral forms that can be integrated with GP clinical systems
- Information for patients (including resources in other languages and formats, and the Health information Leaflet Service catalogue)
