What is this all about?

Many people living with a long term condition tell us they would like more control over their health and the care they receive. They also want more support in being able to care for themselves (self care). Personal Health Planning helps you to have more control.

What is a Personal Health Plan?

A Personal Health Plan helps you plan your needs. You may already have a care plan, but you may want to develop your plan by setting your health goals and organising your health details etc. The individual involved in your care can advise you.

Who can have a plan?

Anyone can have a plan; however, it is currently being offered to patients with chronic obstructive pulmonary disorder (COPD) or heart failure. From September 2010 plans will also be offered to people with diabetes, dementia and multiple sclerosis.

How do I use a plan?

You can decide what information you want to include, and you can complete it in stages or all in one go, as much or as little as you wish. It’s also likely you will need to change the Information you include over time.
