Personalisation Agenda: Raising awareness
Project Summary:
‘The person first, the condition second: An introduction to personalisation’, booklet and short film was produced in order to explore what personalisation means for Norfolk and Waveney.
Personalisation is an approach to health and social care which puts the person first and their condition second. It recognises people’s own expertise by reinforcing the idea that individuals are based placed to identify their own needs and the most effective ways in which these can be met.
Co-produced with service users from the Norfolk Coalition of Disabled People (NCODP), the guide outlines some of the key initiatives taking place, such as personal health planning and personal budget. The short film that accompanies the guide is a powerful illustration of how personalisation translates into everyday life for a variety of people.
